Dr. Abbott received her medical degree from Michigan State University,
College of Human Medicine in 1990, and completed her residency
training at St. Mary's Hospital in Grand Rapids, Michigan, in
1993. She is board certified in Family Medicine, and was in private
practice in Lakewood, Colorado for seven years.

Dr. Alexander is a Professor in the Department of Family Medicine.
In addition to teaching and practicing, she serves as
MSU's University Physician. This is the health officer for the
University, answering to the Provost and Vice President for Financial Operations. In her role as University Physician,
she in charge of Pandemic Flu planning for the University.
She was awarded the 2007 CHM Outstanding Clinician Award for her service throughout MSU and the community. She has won awards as Outstanding Woman Educator in the State of Michigan and as Outstanding Woman Faculty at MSU.
Her main current research project is entitled "Healthy
E-mail" which focuses on getting evidence based summaries
of research on common health issues to patients and physicians
at the same time.
Dr. Alexander received her medical degree from the University
of Kansas (1977), served her family medicine residency at Wesley
Hospital in Wichita, Kansas, and is Board Certified in Family Medicine. She has additional training in family therapy,
sexual dysfunction counseling and adolescent medicine.

Hend Azhary, MD
Dr. Azhary is an Assistant Professor in the Department of Family Medicine. She sees patients at the MSU Family Medicine Clinic and at the Sparrow/MSU Family Medicine Residency Program's Mason Clinic. She is also engaged in teaching CHM medical students. Dr. Azhary most recently practiced as a family physician in Hereford, Texas. She received her medical degree from Tishreen University in Syria. Her residency education in family medicine was at the State University of New York, Buffalo, where she also completed a geriatric fellowship. Her research interests focus on diabetes and cardiovascular studies.
Dr. Barry is an Associate Professor and Associate Chair for Research in the Department of Family Medicine, College of Human Medicine. He received his medical degree from the
University of Maryland (1982), served his residency at St. Lawrence Hospital, and is Board Certified in Family Medicine. In 1995, Dr. Barry completed a masters degree in clinical research design and statistical analysis at the University of Michigan School of Public Health. Dr. Barry's interests include information use in clinical settings, evidence-based medicine, and clinical decision making.

Karen Blackman, MD
Dr. Blackman is an Assistant Professor; seeing patients at the Family Medicine Clinic and at Olin Health Center. She also teaches in the Sparrow/MSU Family Medicine Residency Program. Her position allows her to utilize her two specialties, Family Medicine and psychiatry, in an effort to bring psychiatric knowledge to primary care physicians. Dr. Blackman received her medical degree from Michigan State University College of Human Medicine. She graduated from the MSU/St. Lawrence Family Medicine Residency, then worked as a family doctor, and later, completed a second residency in psychiatry here at MSU. From that time, she was a general psychiatrist in East Lansing, until joining the faculty in 2005. Dr. Blackman is board certified in both family medicine and psychiatry.
Robin H. DeMuth, MD
Robin H. DeMuth, MD, joins the East Lansing campus of the Department of Family Medicine as an Assistant Professor, HP, with primary responsibilities in teaching clinical skills at the medical student level. She will also have clinical responsibilities and will participate in scholarly activities and special assignments. She earned her BA at Rice University in Texas and received her MD from the MSU College of Human Medicine in 2004. As a graduating medical student, she received the David Hough, MD, Award for excellence in family medicine. She is a 2007 graduate of the Sparrow/MSU Family Medicine Residency Program in Lansing, where she followed the Obstetrics Track. Dr. DeMuth has served as a Resident Clinical Instructor for CHM, precepting with medical students on history/physical exam skills, working with clerkship students on in-patient rounds and out-patient clinics, and preparing scenarios and exam questions for student evaluations.

Lynda J. Farquhar, PhD is Professor in both the Department of
Family Medicine and in the Office of Medical Education, Research
and Development. She received her PhD in Higher Education Administration
from MSU in 1979. She currently serves as Director of Graduate
Medical Education for the College of Human Medicine. In that role
she is responsible to maintain College accreditation for College
sponsored residency programs. She also serves as the Contract
Officer for affiliation agreements with GME programs across the
College's affiliated communities. In addition, she serves as the
Assistant Dean for graduate students in CHM, and as Director of
the Medical Scientist Training Program. She also directs the Office
of Continuing Medical Education. Dr. Farquhar's major academic
interests are medical education and curriculum development. She

Barbara S. Garvey is the administrator for the departments of Family Medicine
and Pediatrics & Human Development. Her original professional
focus was as an intensive care nurse, with a Master of Science
in Nursing from Wayne State University. In 1979 she began a move
into administration, as nurse manager of the MSU College of Osteopathic
Medicine specialty clinics. As Garvey obtained a Masters in Business
Administration from MSU, she also moved to administrative positions
with MSU Human Health Programs, and in 1994 became Managed Care
Administrator for the MSU HealthTeam. Her most recent position
with the HealthTeam was Business Analysis Unit Administrator.
Her emphases have been in creating new
departments and programs, developing business plans, providing
financial analysis, and helping to build automated systems that
streamline the tracking and maintenance of administrative and
financial data for medical practices.

Dr. Gerard is founding chair and professor of Department of Family Medicine. He received his medical degree from the University of
Michigan in 1953 and is board certified in Family Medicine. He
is a fellow of the American Academy of Family Physicians and president
of the Capital Chapter, Michigan Academy of Family Physicians.
In 1991 he received both the "Educator of the Year" and
"Life Time Achievement" awards from the Michigan Academy
of Family Physicians.

Dr. Given is Professor in the Department of Family Medicine.
He also holds a research scientist position through the Walther
Cancer Institute. His research interests include the study
of home care and the support of caregivers for cancer patients.

Raza Haque, MD
Raza Haque, MD, joins the East Lansing campus of the Department of Family Medicine as an Assistant Professor, HP. He will perform clinical work, geriatric assessments, skilled nursing facility care and medical directorships. He will be Associate Director of the geriatric fellowship; teach at the medical student, resident and fellowship levels; and participate in other scholarly activities. He earned his medical degree at Dow Medical College, University of Karachi. This was followed by a residency in Internal Medicine at Oakwood Hospital in Dearborn, and completion of the Geriatric Fellowship at MSU in 2004. He is a Diplomat of the American Board of Internal Medicine, a Fellow of the American Geriatrics Society, and has a certificate of added qualification (CAQ) in Geriatric Medicine. Dr. Haque is Medical Director of Faith Haven Senior Care Center and offers subacute care at the Ingham County Extended Care Facility. He has a scholarly interest in preventing polypharmacy and falls in the elderly.
Jodi Holtrop, PhD, CHES, is an Assistant Professor with a tenure stream position. She will focus on research in this position related to health promotion and disease prevention, and has relinquished her role as director of the Family Medicine Residency Network. Dr. Holtrop is interested in redesigning primary care practices to incorporate better intervention delivery. She has a special interest in developing strategies to encourage positive patient health behaviors such as smoking cessation. Dr. Holtrops’s current areas of research related to primary care include integrating technology and practice support for comprehensive preventive care delivery.
Dr. Holtrop received her PhD in health education and educational psychology from The University of Toledo in 1996. She is a professional health educator and has maintained certification as a Certified Health Education Specialist (CHES) since 1990. Her research interests include transforming primary care medical practice to effectively deliver health promotion and disease prevention, health behavior change strategies, and smoking cessation. She serves as principal investigator or co-investigator on several funded research projects. Other activities in her role in the department include coordinator for the research and scholarly projects curriculum at the Sparrow Family Medicine Residency Program, where she mentors family medicine residents on their research studies, and teaching and course/curriculum development in blocks I and II of the MSU College of Human Medicine medical school program.

Gregory S. Holzman, MD, MPH
Greg Holzman, MD, MPH, joins the East Lansing campus of the Department of Family Medicine part-time as an Associate Professor. He will practice once a week in the Family Health Center and will teach at the medical student level. He earned his BS from MSU and received his MD from the University of Florida’s School of Medicine. After completing a residency with the Carolina Medical Center Department of Family Practice, he earned an MPH and completed a Preventive Medicine Residency at the University of Washington. He is board certified in family medicine and preventive medicine. Dr. Holzman is currently the Chief Medical Executive for the State of Michigan. Prior to accepting this position, he was an Associate Professor in the University of North Dakota School of Medicine and Health Services.
David M. Kozishek, MA, BCC

David Kozishek, BCC, MA, joins the of the East Lansing campus Department of Family Medicine and the Center for Ethics and Humanities as an Assistant Professor. He will be responsible for providing leadership in the spirituality and medicine components of medical student and resident training at the College of Human Medicine and will assist with related teaching needs in bioethics professionalism and the patient-physician relationship. He will develop and offer training programs in pastoral care for clergy in the areas served by CHM’s six community partners, and will develop and engage in scholarship related to the practice and teaching of spirituality and medicine. He earned his MA in Religious Studies at the University of Louvain in Belgium. Currently Mr. Kozishek is a PhD candidate at the University of Texas Institute for the Medical Humanities. He is also a Board Certified Chaplain (BCC) through the Association of Professional Chaplains. His dissertation topic is “A Hermeneutic Analysis of Clinical Ethics Practice in the Twenty-First Century: Toward a Practice Grounded in the Humanist Tradition.”

Dr. Kuhn received his medical degree from Heidelberg University in Germany in 1987. He spent a year in research,
followed by his mandatory service in the German Army. He then joined the Emergency Department at the United States
military hospital in Heidelberg for five years. Dr. Kuhn moved to the United States and completed his family medicine
residency at St. John's Hospital in Detroit, Michigan. He is board certified. After spending one year in Lansing practicing
urgent care, he completed a fellowship in obstetrics at Case Western Reserve University. He joined our faculty in 2001.
He completed the MSU Primary Care Faculty Development Fellowship Program and then earned his Master in Business Administration.
Dr. Kuhn's academic interests are obstetric training for family medicine residents and developing new marketing strategies
for primary care.

Rebecca A. Malouin, PhD, MPH
Dr. Malouin is an Assistant Professor and Director of International Programs within the Department of Family Medicine. As Director, she is the lead administrator for the Methods in Family Medicine Education Training Program, a program supporting the development of primary care and family medicine in Japan by providing 6-12 months of individualized training in family medicine education to Japanese clinicians within the department. Dr. Malouin’s research interests include systems and supports to promote primary care and the patient-centered medical home, both within the United States and abroad. Dr. Malouin has methodological interests in needs assessments, program evaluation and mixed methods approaches in research. Dr. Malouin also provides instruction and course/curriculum development within the College of Human Medicine, particularly in areas related to clinical skills and global health.
Dr. Malouin received her MPH (1997) and PhD (2002) from the Johns Hopkins Bloomberg School of Public Health and is completing a postdoctoral fellowship in the Michigan State University Department of Epidemiology. Dr. Malouin served as a Peace Corps volunteer in rural Niger, West Africa (1994-1996) and has served as an epidemiologist in both the Maryland Department of Health and Mental Hygiene (1997-1998) and the Michigan Department of Community Health (2001-2005). Dr. Malouin currently holds a joint position with the Department of Pediatrics and Human Development.
Dr. Noel is a Professor and Senior Associate Chair for the Department of Family Medicine. She received her PhD in Family
Ecology from Michigan State University in 1988, specializing in
nutrition, management, education, family, and Certificate for
Aging Studies. She is a registered dietitian. She received the
Blue Cross Blue Shield of Michigan Foundation Excellence in Research
Award in the area of Clinical Research in 1998. On January 1, 2007, she assumed the position of Senior Associate Chair in the Department of Family Medicine of Michigan State University's College of Human Medicine (CHM). Dr. Noel, accepted the appointment after serving 10 years as the department's Associate Chair for Academic Affairs.

Julie P. Phillips, MD, MPH
Julie P. Phillips, MD, MPH, joins the East Lansing campus of the Department of Family Medicine in a half-time capacity as an Assistant Professor, HP. Her primary responsibilities involve clinical practice, including OB; along with teaching at the medical student and resident levels. She will also be involved in research and other scholarly activities. After earning her BA at Albion College, she received both her MD and an MPH in Health Management and Policy at the University of Michigan in 2004. She completed her residency in Family Medicine at the University of Michigan. She is currently principal investigator for an ongoing research project on medical student perceptions of physician work life, and its impact on specialty choice. In 2006, she received the William Clippert Gorenflo Research Award from the UM Department of Family Medicine in recognition of her academic achievement, passion for research and an outstanding research project in Family Medicine.
Dr. Ogle is a Professor in the Department of Family Medicine. She is board certified in Family Medicine with a Certificate of Added Qualifications in Geriatrics and is also Board certified in Hospice and Palliative Medicine. She directs the Palliative Care Consult Service at Ingham Regional Medical Center. She has developed "Easing Cancer Pain: A Fireside Retreat" (www.easingcancerpain.msu.edu) and “Completing a Life” (www.completingalife.msu.edu), resources for patients and families on cancer pain and end-of-life care.

Dr. Pathak is a Professor in the Department of Family Medicine. She
received her PhD in statistics at the University of New Mexico,
Albuquerque, New Mexico, in 1975. She has also earned a MS in
epidemiology from the Harvard School of Public Health.

Dr. Pearson is an Associate Professor in the Department of Family Medicine. He is the Senior Associate Director of the Sparrow/MSU Family Medicine Residency, Director of the MSU Sports Medicine Fellowship and Team Physician for MSU Football. He received his MD from Wayne State University
School of Medicine in 1981 and has been a fellow in the MSU Family
Medicine Faculty Development Fellowship Program. He is board certified
in Family Medicine with a certificate of added qualification in
sports medicine.
Dr. Pearson is a fellow of the American Academy of Family Physicians. He served as physician for the US delegation to the 1999 World University Games in Mallorca, Spain and has served as the Medical Director of the Michigan Breast Cancer Three-Day for the past four years.

Brian Z. Rayala, MD
Dr. Rayala completed his residency through the Sparrow/MSU Family Medicine Residency Program, where he served as Chief Resident during his senior year. As Assistant Professor, he practices at the MSU Family Medicine Clinic, teach residents in the Sparrow/MSU Residency Program, and teachs medical students as Associate Director of the Clinical Skills Program. Dr. Rayala received both his bachelor of science and medical degree from the University of the Philippines Manila College of Medicine through their combined, accelerated INTARMED (BS-MD) Program.
Carlos F. Ríos-Bedoya, ScD
Carlos F. Rios-Bedoya, ScD, joins the East Lansing campus of the Department of Family Medicine as an Assistant Professor with a research focus on adolescent risk assessment within both local and international health settings. His research will link with departmental themes in health promotion, disease prevention, and outcomes assessment research. He will also share in teaching responsibilities. After earning his BS and MPH at the University of Puerto Rico, Dr. Rios-Bedoya received a ScD in psychiatric epidemiology from Johns Hopkins University in 1999. Most recently he has served as a post-doctoral Fellow of the National Institute on Drug Abuse. From 2004-2006, he pursued his work as a Fellow at Johns Hopkins University Department of Mental Health, and since that time has been located at the MSU Department in Epidemiology. His recent research has focused on the association between childhood risk-taking behavior and subsequent drug use as a young adult. He has published extensively in the areas of drug use, alcohol use and HIV infection.
Steven E. Roskos, MD

Steven E. Roskos, MD, joins the East Lansing campus of the Department of Family Medicine as an Associate Professor, HP. His primary responsibility is leading the Clinical Skills curriculum for the College of Human Medicine. Clinical practice and scholarly activities will also be part of his responsibilities. He earned his BS at Wheaton College and received his MD from Temple University School of Medicine in Philadelphia. This was followed by a residency in Family Medicine at Lancaster General Hospital in Pennsylvania. He completed the Primary Care Faculty Development Fellowship here at CHM, with an emphasis on the Curriculum Development Track. In this fellowship, he developed a curriculum in office procedures for family medicine residents. He is board certified in Family Medicine. Dr. Roskos comes from a faculty position at the University of Tennessee Graduate School of Medicine. A focus of his research has been patient medical literacy and consumer medical instructions.
Dr. Roskos won the Distinguished Paper Award at the 40th annual STFM spring conference in 2007. The paper is Roskos SE, Keenum AJ, Newman LM, Wallace LS. Dr. Smith is an Associate Professor in the Department of Family Medicine,
College of Human Medicine. She received her medical degree from
the University of Michigan Medical School (1980), served her residency
at Southern Illinois University, Memorial Hospital of Carbondale,
and completed a masters degree in research design and data analysis
at the University of Michigan, School of Public Health in 1986.
Dr. Smith is Board Certified in Family Medicine. Dr. Smith's interests
include obstetrics and gynecology, primary care, rural health
care, and developing writing and research techniques and programs
for residents and new authors.

Dr. Sorkin is an Assistant Professor and assistant director of the
Michigan State University Sparrow Hospital/St. Lawrence Campus/Family Medicine Residency in Lansing.
She received her medical degree
from the University of Pittsburgh School of Medicine and has participated
in the Faculty Development Fellowship Office of Medical Education,
Research and Development (OMERAD), Michigan State University.
She is a board certified family practitioner.

Dr. Wadland is Professor and Chair of the Department of Family Medicine which is heavily invested in advancing education, scholarship, and clinical care. Over 35% of the department’s efforts are funded from extramural research funds including significant NIH funding for a clinical department. He personally has been principal investigator for funded studies ranging from medical education, humanities and medicine, to numerous prevention trials on smoking cessation. He is particularly interested in systems that help clinicians incorporate health promotion and disease prevention programs into their busy practices. He has advanced masters training in research design and statistical analysis and has served as a mentor on numerous research projects for faculty, resident physicians, and students. He is also the Associate Dean for Faculty Affairs and Development for the College of Human Medicine providing opportunities that promote faculty vitality such as programs on leadership, faculty mentorship, core competencies in medical education, and the key elements for successful faculty advancement.
A paper by Dr. Wadland et al was cited as a high impact publication in the field of Computerized Clinical Decision Support in the Year in Review session at the American Medical Informatics Association Symposium, November 12, 2007. David T. Walsworth, MD, FAAFP
Dr. Walsworth is Associate Chair for Clinical Affairs and Medical Director of the Family Medicine Clinic, with the academic position of Assistant Professor. He comes to MSU from Oakwood, where he was Vice Chief of Family Medicine at Oakwood Annapolis Hospital, Associate Program Director of the Family Medicine Residency Program, and Unit Medical Director of their Healthcare Center in Westland. He received his medical
degree at Wayne State University School of Medicine and completed his family practice residency at Oakwood Hospital &
Medical Center, Dearborn. He then completed the OMERAD Primary Care Faculty Development Fellowship in 2002. He serves on
the board of directors for both the Michigan State Medical Society and the Michigan Academy of Family Physicians.
Dr. Weismantel is an Associate Professor in the Department of
Family Medicine. He completed his family medicine residency at
the Mayo Graduate School of Medicine. In 1997-98, he was assistant
director of the Scottsdale Healthcare Family Practice Residency
Program and was a clinical Assistant Professor of family and community
medicine for the University of Arizona College of Medicine.

Dr. WinklerPrins is the Associate Chair for Academic Affairs and an Associate Professor in the Department of Family Medicine. Since joining the faculty,
he has coordinated the Methods in Family Medicine Education Training Program which supports the development of primary care in
Japan. He has been board member for the Family Practice Inquiries Network (FPIN), an organization which identifies the clinical
questions that primary care doctors need answers to and then develops the evidence-based answers to those questions. He is the
current editor-in-chief of PEPID-PCP, an electronic textbook of primary care medicine, and is the recipient of the 2006 College of
Human Medicine Outstanding Clinician Award. In 2007, he was awarded membership in Alpha Omega Alpha, the national honor medical society.
He is a Michigan native. He received his medical degree from the University of Michigan in 1986, completed his specialty training
in Family Medicine at the University of Connecticut in 1989, and is board certified in Family Medicine. He joined the Department of
Family Medicine at MSU in 2000 after nine years of mostly rural practice in Wisconsin. He spent 1999-2000 in The
Netherlands as an asthma researcher, and lived and worked in the Brazilian Amazon for six months as a physician volunteer.
His special medical interests include international health, health care delivery, medical education and pediatric health.
